The form need not be submitted. It is only a guide to help applicants prepare information prior to applying online.

For foreign corporate investor, supplement:

Notarized copy of Certificate of incorporation of the foreign investing company (an authentic copy)
or equivalent. Documents not in English, to be translated and notarised by an authorized organization in the investor's country

Shareholder's resolution of the foreign investing company (original)

Audited accounts of foreign investing company for the last financial year (Simple copy)
